California Workers' Compensation Guide

Official Medical Fee Schedule (OMFS) Calculator

What is the OMFS Calculator?

Under California Labor Code Section 5307.1, the Official Medical Fee Schedule (OMFS) establishes maximum reasonable fees for medical services provided to injured workers. The OMFS Calculator allows medical providers, claims administrators, and attorneys to calculate accurate reimbursement rates based on state-mandated fee formulas.

Step 1: Required Information for Calculation

To determine the correct maximum allowance for a medical bill, ensure you have the following essential data points:

  • Date of Service (DOS): Reimbursement rates are calculated using the specific fee schedule rules in effect on the exact date service was provided.
  • Place of Service Locality: Fees vary based on the geographic locality (County or Zip Code) where care was rendered (CMS-1500 Box 32).
  • Procedure Codes: Standard CPT, HCPCS, or California-specific codes representing the medical services provided.
  • Facility vs. Non-Facility Setting: Determines whether services were provided in an office setting versus a hospital or ambulatory surgical center.

Step 2: OMFS Fee Schedule Components

The OMFS applies distinct payment methodologies across different medical categories:

Schedule Category Calculation Basis Key Rules / Modifiers
Physician Services Resource-Based Relative Value Scale (RBRVS) using RVUs & Geographic Practice Cost Indexes (GPCI). Adjusted by Provider Type (Physicians @ 100%, Non-Physician Practitioners @ 85%).
Pharmaceuticals Medi-Cal Rate + $7.25 Professional Dispensing Fee per NDC/Date of Service. Calculated at lower of Medi-Cal price or provider's Usual & Customary charge.
Medical-Legal Fixed state fee codes (e.g., ML201, ML202) + hourly or modifier-based rates. Governed by California Medical-Legal Fee Schedule regulations (ยง 9795).
Outpatient & Facility California-adjusted Medicare Ambulatory Payment Classifications (APC) or DRGs. Applies facility-specific cost-to-charge ratios and area wage indexes.

Step 3: Calculating Fee Schedule Adjustments

Once base rates are established, standard statutory rules are applied to reach the final Maximum Reasonable Fee:

  1. Fee-Affecting Modifiers: Apply relevant CPT/HCPCS modifiers (such as -26 for professional component, -TC for technical component, or -51 for multiple procedures).
  2. Multiple Procedure Discounting: Secondary and subsequent surgical or diagnostic procedures are discounted according to RBRVS ground rules.
  3. Provider Adjustments: Verify whether the servicing clinician is a licensed physician, PA, NP, or physical therapist to apply statutory percentage factors.

Step 4: Handling Bill Disputes & Explanations of Review

If an insurer reimburses below the calculated OMFS rate, take action immediately:

  • Explanation of Review (EOR): Review the carrier's EOR for specific reduction codes or rationale.
  • Second Review Request: Submit a formal Request for Second Review within 90 days of receiving an underpaid EOR.
  • Independent Bill Review (IBR): If the second review does not resolve the fee dispute, file for Independent Bill Review (IBR) through the DWC within 30 days.
